What is calf augmentation?

Calf augmentation is a lower limb contouring plastic surgery that utilizes silicone implants or fat grafting to add volume and definition to thin legs.

Our philosophy prioritizes precise anatomical subfascial placement. Rather than placing the implant loosely beneath the skin, a precise surgical pocket is created immediately under the fascia of the gastrocnemius muscle (inner calf). This provides a protective muscular cushion that conceals the implant to the touch and prevents unwanted rotation during movement.

The goal is to achieve a toned, muscular contour in the calves, correcting constitutional imbalances (thin legs resistant to hypertrophy at the gym) or prior muscle volume deficiencies.

POSTOPERATIVE

Recovery timeline

Walking during the first few days may be uncomfortable due to tightness in the calf; relaxing the muscles with wedge footwear is essential.

Days 1 to 3 — Bed rest and leg elevation

Calves will feel tight and stiff, similar to the soreness after extreme physical exertion. You must rest in bed and keep your legs elevated on pillows.

Days 7 to 10 — Removal of popliteal sutures

Sutures in the fold behind the knee will be removed. You can begin taking longer, smoother steps while wearing 2 cm wedge shoes.

Week 3 — Return to work and normal walking

Resume light office work and short walks without special footwear. Compression stockings must continue to be worn during the day.

Week 6 — Athletic clearance and stabilized volume

Tissues have fully healed and integrated. Gym workouts, running routines, and weighted leg exercises may be resumed.

Common questions about calf implants

The incision is made strictly within the natural fold behind the knee (popliteal crease). This provides excellent camouflage, making the scar nearly invisible when the patient is standing. Over the following months, the mark fades and blends naturally with the normal folds of the knee.
You can walk very short, slow distances inside your home starting from the second postoperative day. However, walking smoothly and without muscle tightness is typically achieved between 10 and 14 days. Wearing shoes with a low heel or wedge (2-3 cm) is recommended during the first few weeks to keep the calf muscles relaxed.
Calf implants are made of a soft, highly cohesive solid silicone elastomer, specifically designed to mimic the natural firmness and elasticity of a toned calf muscle. Unlike breast implants, these are solid and extremely durable, so they do not pose a risk of leaking or rupturing from ordinary physical impact.
They are placed beneath the fascia of the gastrocnemius muscle (inner calf), which provides excellent coverage of muscle and subcutaneous tissue over the implant. This prevents the implant from being palpable to the touch or visible as a foreign body, achieving a highly natural transition.
Any high-impact physical exercise involving the legs (such as running, jumping, heavy cycling, or weightlifting) must be suspended during the first 6 weeks following surgery. This ensures the stable fixation of the implant within the muscular pocket and prevents secondary displacement.
